I don't think that I pruned it hard enough last winter. I tend to prune in the Summer but the shoots are now about 12 inches long and if I cut back to about four nodes there would be no leaves left!
I don't see a reason to prune, at this time, unless you want to shorten the primary branches.
Here are some articles that will help you to know when and how to do this:

Orange growth noticed at base of trunk and bark very dark and cracked on old cooking apple tree (at very least 80 years old). Photos I hope show this. Leaves look very good this year and blossomed well but don’t seem to be many apples starting. Yield has decreased over past couple of years.
That is extraordinarily old for an apple tree! That should be in shambles by now.
There is nothing that you can do for this tree, besides continue proper care, until its death.

Our 1st year Apple tree has small apples on it but the leaves are turning yellow. We are afraid that it is dying. If we trim the limbs back will that help it?What can we do to help it?
Unless you have an apple that is well suited for the South, it may be too hot for the tree. I've included a link that explains how and when to prune.

I was given an apple tree as a present Malus Malibu Subito It is growing and there is an apple On it. The problem is there is something growing alongside it in the pot and I don’t know if it is part of the tree. The leaves look slightly different The apple tree leaves are more elongated. Should I cut them off ?
These are suckers from the root, which is a completely different Apple variety. Keep them trimmed, and correct any environmental factors that aren't quite where they should be to keep these from occurring.

Neither my nor my neighbours, admittedly older than ourselves, apple trees produced much, if any blossom, and so no/little fruit has set this year. Any ideas why ?
Apples have a tendency to stop production, quite suddenly, once they reach a certain age. This is usually around 40 or 50 years. This can be earlier or later, though.
This is my suspicion.
Other than that, proper care will keep them at their best.

This is one of two trees bought together but different types of apple. One is fine but the other has some yellow leaves. Next to each other on the same sunny wall.
This can be normal, but it can be a sign of overwatering as well. You should make sure to let the soil dry out completely down to about 2 or 3 inches or so between waterings. This will keep any infections from starting in, which can kill your tree.

Could you confirm if this could be anything other than fire blight on my apple tree. It is pretty much all over the tree. I have sprayed it with bug and mildew spray but from reading your website I think it should have been a bacterial one. I have also fed it some potash. It didn't blossom long and there is very little fruit. I have a pear tree beside it that only has slight infection and my neighbour's overgrown trees x 2 has some too. If I have to cut it out it will have to be cut back to the trunk so probably no point. Hope you can give me some hope.
That does appear to be fireblight, unfortunately.
You can try cutting out the diseased branches on the one with only slight infection.
