I have a foxtail,15 feet to the frown base that is losing its bark like exterior about 5 feet off the ground. Is this normal? Should I do something to protect this area?
This can be normal for really old frond remnants to start chipping. However, if you are noticing signs of disease in the living fronds, then there may be some concern.

Palm was doing great, then it was like the top center frond got stuck in trunk, didn’t come out all the way, new ones come out but only a little way, then they try to open up, but only half is out of the trunk.
Heavy rain, saturated soils, or a chemical (boron) deficiency can cause fronds not to open. They are heavy feeders and also need lots of water, but soil must be well drained. Make sure the fertilizer you use contains micronutrients. Here is more:

Since about 2 months ago, my foxtail palm tree started to show signs of what seems to be a disease. The fronds have been getting dry and wilt, starting from the trunk, towards the tip. The tree is 4 years old only.
It could be one of these issues in the article below. I can't tell from the photo if there are leaf spots, but you can examine the fronds more closely. I've also included an article on general care.
