My Forsythia is 2 yrs. old. Flowered this spring but the leaves started to die off soon after. I pruned the branches back but it is still dying. Any suggestions?

Forsythia are touch and generally disease free. Check the soil drainage and amount of water - boggy conditions can definitely cause weakness and dieback. Also too much shade could do it.
Can anyone tell me what this plant is please? When purchased at beginning of year, it appeared to have a stem, yet is growing from the ground. It also had yellow 'blooms' on it.
Sheila, I can't quite make out the shape of the leaves, but it resembles a Forsythia which has small yellow flowers.
I treat my forsythia the same every year. I prune after it has flowered but this year it looks dead and the branches are very brittle.
Unfortunately sometimes you can't explain the loss of a healthy plant.
Forsythia are quite hardy, and my recommendation if you see no leafing out of the plant, would be to trim it down.
The roots may still be alive and then have the energy to start your plant growing again. I never give up on plants until I'm sure.

They only have a few blooms on each bush but they green out fine. This has happened the last two years.
There could be several reasons for your issues.
Here is an excellent article that may help you pinpoint your lack of flowers.

I have several forsythias. They have long shoots. Almost resemble a cane pole, shooting out 8-10ft. into the air. Prolific blooms, but the plants look obnoxious. My neighbor has a forsythia hedge. The variety he has doesn't have the long shoots. It's just a nicely shaped hedge. Never see him prune it. What are the different varieties? I like the way his looks and would like to purchase this type. Thank you....Bruce
This article should help you as it describes all the varieties of forsythia, from large, mid-size to small:

The forsythia bush hasn't bloomed in the last two years. I prune it after it has bloomed, but maybe I'm waiting too long after it's done blooming, usually a month or more.
This article will help you determine the cause:

The best time for trimming forsythia is in the spring as soon as possible after the blooms fade. Forsythia sets flower buds in late summer on the growth it puts on during that years growing season. Hence, blooms are created on one year old wood.

Can you plant a forsythia plant before the long weekend in May? I just bought a forsythia shrub and I wasn't sure if I should plant it now or wait for the danger of frost to be gone.
You can plant your Forsythia now.
Here is a link to proper growing conditions.
Water the new plantings well to ensure success.
