What is the new growth above my bottle?

Question

I have a large tube-like extension growing out of the tree, right above the bottle, below the fronds. What is it? Do I do anything about it?

Nope! You don't need to interfere at all. These are flowers, and will drop off once they are done blooming, and producing seeds, if they do.
