Why is my ponytail palm breaking off its upper leaves? Should I transplant my ponytail palm? Please see photos
It appears that the plant may be rootbound! This can be remedied by repotting. This article will help:

As far as what has cut the top off- That will be hard to say without catching the culprit in the act, unfortunately.
Here is an article that will help you with general care of the plant:

I was gone for a few months and my caretaker severely overwater my ponytail palm causing it to get root rot and its almost dead. Is there any way to save it? It has no pups, but has several tufts on top.
Did you try repotting it in fresh soil and clean container? If the bulb is mushy, there is probably no hope, but if the top still looks good, I would cut off a tuft close to the trunk or as far as healthy wood, and let it callous for two or three days. Then dip it in rooting hormone and plant it in a well-draining cactus soil to root.

my two plants are 30+ years old; they both have growth on the main stem-- are these pups??? if so how do you propagate them?? help!!
Yes, they are. These articles should help:

Ponytail palm was left outside in cold weather and the leaves started to die so we stripped the leaves and brought the plant inside. We thought it would die but after several weeks growth appeared where the leaves had been attached to the stem. To our surprise the growth turned out to be pups growing out where leaves should appear. What do we do? Should we remove the stem at the base? Please see photos.
Those are branches, rather than pups! This is what happens, naturally, when a plant is stressed just the right way. There is nothing to be done at this time.
Enjoy the more full appearance when it is recovered!!!
. I have attached a photo of the problem palm. Can please advice what may be causing this
Without more information it is hard to judge. I do see it has to compete for nutrients with the other plants. Is the other tree planted among other shrubs?
ALso, you may be watering those other shrubs more than the ponytail palm needs. I notice references say it should be planted as a specimen tree.
Here are its care requirements outdoors:

The tree looks fine otherwise, so i didn’t notice this separation right away. Worried, as there are 2 more inclose proximity.
This sometimes happens when extreme conditions fluctuate, like dry and suddenly wet, or extremely hot and suddenly cold, etc. Here's an article that should help:

Palm is about 15 ft tall, growing in the garden
You could give it a try. Here is how:
