Unknown Pear Tree Disease

I have a pear tree that is about 15-20 yrs old. For several years now the pears begin looking very good then soon begin to slowly turn black and crack deeply. What is this disease and how do I treat it??

Pear scab and Fabraea leaf spot can have those symptoms. Also possible are fire blight or black rot. These articles should help.
