Replanting during times of stress, without curing the issue at hand can result in the plant's death.
You will have to let it recover after treating with a fungicide, and make sure not to overwater. This is what likely caused the issue in the first place, as these can suffer pretty badly when overwatered. Their succulent leaves are designed to hold water, so you want the soil to dry out down to 2 or 3 inches between watering. This does not count the rocks on top, so make sure to test the soil.
