Borer Worms in Zucchini

Question

My zucchini plant is dying overnight. After inspection I found white worms with a black (head?). I cut into the trail and took out the worms and am going to replant with some root generator to see what happens. How can I keep these worms away, and where did they come from? They don’t bother any other plant (for now anyway).
