If the leaves are falling off I'd be concerned about root rot. Once a month watering may be too much in this case. Wait until the soil is very dry a couple inches down in the pot. I would go ahead and repot in clean container and new soil, preferably labeled for cacti/succulents. Also be sure it is getting lots of light, such as a south or west window.
