What causes squash to grow maybe 3″ and then just dry up and die?

I have a raised garden bed that is two feet high. Lots of soil and limbs. Is it possible that the soil is not staying moist?

This could be a couple of different issues; possible Blossom End Rot or poor pollination.

These articles cover these issues.
