Creeping Phlox Has A “mushroom” Like Growth Among The Stems.

Growth seems to be on the plant and not from the ground. What could cause this and what do I do?

It looks to be a Black Staining Polypore, or Meripilus species. It is consuming the wood in your bed, and doesn't really pose a risk to plants. You can remove it if you choose.
