Generally, they are cut just above the knuckles. Prune all but the straightest of the small branches emerging from the knuckles, leaving about 4 inches above the knuckles themselves. These straight branches should line up with the largest part of the branch below the knuckle. As they gradually thicken and grow, the knobby joints will become less noticeable.
