Zucchini Plant Leaves

Question

We have a new looking leaf on our plant. The stem is long with spike-looking leaves. The top leaves are not normal. They are curled with two different colors. The flowers are falling off and the fruit does not grow more than an inch long. Is this due to pests or something else?
