Q.Why Is My Cactus Shriveling?

I just bought this cactus last week and I just tried to give it a spoonful of water yesterday. I just noticed that it shows a wrinkle in the body. The soil is still damp from previous watering and we’re currently in the rainy season. I’m wondering if my cactus is okay? Should I add more water to the pot?

No. This is being overwatered. Cacti need the soil to dry out VERY thoroughly to avoid issues like this. The plant's roots are suffocating.
