That's a fungal infection and needs to be treated ASAP. It is likely Alternaria. Even with treatment, it will likely kill any tomato that has already been infected enough to show these symptoms. Some are resistant, however.
You will need to treat with a fungicide, but most importantly, watering habits need to be corrected. Overwatering is how this disease is spread. It cannot survive in dry soils, so it is important to let soil dry out thoroughly down to about 3 inches between waterings.
