    It's hard to diagnose a plant problem without a photo and more information on the soil and water management program.
    First, consider whether you could be watering too much, or too little. Dig into the soil surface with a finger and assess the subsurface moisture content. If it's dry you may need to water. If it's muddy wet or if there is water standing in the saucer, or if there is no drainage holes in the pot, then it could be water logged and the roots are too wet to function or they are decaying.

    The color is likely from both the soil and water, and them mixing.

    You should use a good quality potting soil in your plants.

    This appears to be the start of a fungal infection. The best course of action from here will be that the next time you water, you will replace 1/4 of that with peroxide. You should only need to do this once. It will kill off the infection while leaving your plant alone.

    I did that a tried again, seems i need another idea for this plant? Any suggestions?

    This is common of a fungal infection. You can cure this, since it is early enough. You will need some wettable sulfur, and Dolomitic lime. These two will kill the infection, and balance out the pH of the soil to prevent future infections.
