how do you remove the old moss....
Just carefully remove and unwind the old moss from the orchid roots. You can then repot it with more moss or another orchid potting medium:

The growing tip with the bud has dropped off the new stem on my orchid, will it rectify this or do I need to cut this stem off and hope another one will grow.
Bud drop or bud blast can be caused by a few different things.

My orchid has only leaves but is not having flowers . What can I do?
Most orchids are photoperiodic, and will not flower unless they have a period of over 12 hours (say 14 to 16 hours) of pitch black. You will have to put it into a place for at least 12 hours that it will not receive a single nit of light. They will start flwoering within a few weeks of this. You will have to let them have a "long day" for a few months after flowering, so that it can recover.
I was given a Dancing lady orchid that came in a small container. Do I need to change it into a bigger container?
You will want to pot into an orchid pot; I prefer terra cotta.

This could be injury, but it is more likely a fungal infection.
Replace 1/3 of your water with peroxide the next time that you water. This will help. Also, make sure that you never let it sit in water. Only water these once or twice a week. You can spray them with water, otherwise.

have gone to produce multiple roots and not new flower stalks. I do not over-water and the location near a window is bright,indirect light. What's up?
Did you cut back the existing stalks? Try cutting them back to just above the fourth node and they should flower again. Also, perhaps the plant is looking for more moisture. Try putting it on a dish of pebbles that is filled with water (but below the plant level).
