I have hydrangea bushes where many of the branches are touching the ground. Should I clip/prune them to avoid disease and pests?

You can. You also can cut only the bottom set of leaves. Make sure you don't cut off any buds that are forming for next year (mophead hydrangeas).

You can. You can also only cut the bottom set of leaves instead. Or even see if stem flopping is causing that due to heavy blooms.
