But have large healthy buds on the very tip of the stem plus most of the way down. 1.Can these also be pruned back, if so when is the best time. 2. How can I distinguished between a leaf and flower bud. Thanks

Buds on the tip of the stem should be flower buds, so do not prune those off. You may have a bigleaf hydrangea since it has buds already; they are pruned after flowering. Here is more:
