Poor soil test

I have a small area that I have been growing tomatoes in for several years. I just did a soil test from a kit and here are the results:

PH…7.5
Phosphorus…P2
Potash….depleted
Nitrogen….depleted

My question is what to use to fix this soil for next spring. Can I put fertilizers in this fall? And if so, what do I use? I’m at a loss to know exactly what to do and most searches turn up amounts of most fertilizers with instructions for acres rather then my 8 x 8 plot.

I would start with adding some organic compost to the garden when the garden has been cleaned up from this seasons plants.
Work it into the soil and let the bed set for the winter. Retesting the soil in the spring again prior to planting would be a good idea.
