Q.Onions Fall Over Early

For the last two years, my sweet onions (planted from starts) have not grown nearly as large as the first year I started growing them. The first year I planted them in standard rows at 4-6 inches apart, and they were nice and large way before they started to dry and fall over. The last 2 years, however, I have planted them at 4 inches in a square foot garden setting, and it seems they start falling over long before they have grown full sized, except for some. We have had some windy, stormy times and a lot of 90 degree heat. I can’t figure it out.
