My Mass Cane needs help!!

My Dracaena seems to be dying!! Any help or advice would be much appreciated. She started losing leaves around 6 months ago (the leaves were becoming yellow/brown from the tip side and then going dead one by one) so there are hardly few left. Originally, I thought that I watered it too often, so I started to do a week-10 days water intervals but it didn’t helped. I have noticed that the cane itself became dark brown/blackish at the base where it meets the soil. The soil surface itself is being covered with a layer of like white meshy stuff, which I’m removing every week. I have recently put it in the new pot with some fresh soil mix and I can be sure that there is a bit is a drainage (put some of the little stones as a first layer in the pot) to see if it helps but, unfortunately, there is no result after a month (it actually getting worse and worse). I have attached some pictures showing current condition of the plant. This plant is super important for me so any advice would be much appreciated. Thank you in advance.

Make sure that the pot has a drainage hole, that excess moisture can drain from the container.
Always check the soil, don’t water on a schedule.
It sounds like the soil is wet, I believe your describing mold on the surface of the soil.
