Q.My Madagascar Dragon Tree Is Losing Leaves From Center Stalk

11 yr old plant. Green strong stem, new leaves blooming on all stalks,the center leaves turn soft and yellow in the center of the stalk. I do use tap water and its hard water? This plant was for my husbands sister from her funeral. It’s VERY important I keep it alive. Just noticed drainage holes were clogged with hard water residue. Soil is moisture control, no fertilizer ever. Re potted 4 years ago. Can I re pot now with new soil? Please please help!!!
