My irises are only 5″ tall

They did this last year so I dug them all up and separated them and they are doing the same thing this year

It is very possible that the area does not have enough phosphorus and potassium to sustain proper flowering. Do you fertilize every year? Can you include photos? This can help me to see what is going on a little better.
