Unfortunately, your photos did not come through. I am unable to see the issue at hand. It could be that the mulch is too deep. This can suffocate the roots. Another thing to consider is the source of the mulch, and if it could have been infected with something, already.

Mulch that is soaking wet should be avoided, as it may be a sign that it stays wet.
