I have a jade plant that keeps losing its leaves and one of the stems looks like it has gotten soggy. It is beginning to lean over. What am I doing wrong?

It sounds like the plant is over watered and may possibly have root rot. Cut back on watering. Water only when the soil is dry an inch on top. This article will help you determine if the plant has root rot:
