ill it live? The tree is about 10 years old, about 10′ tall, and about 3″ diameter at the base. There is some damage to the bark at the base (see photo). We are about latitude 45.

It looks like the main tree has died. Those shoots from the root may be a different kind of Oak than the one that you planted if the tree was grafted, but choosing one of the shoots to be the new "leader" and cutting the rest and the dead tree down can help.
Unfortunately I cannot say what has killed the tree. I can include one of our articles for Oak tree care, so that this may be prevented in the future, however.
