Brown rot re-infection can be minimized by sanitation procedures and cutting out cankers during the winter. This article gives more information:

If the tree seems a lost cause, go ahead and have it removed. If you have another location to put a new tree, that would be the best decision. The fungal spores can remain in fallen fruit or mummified fruit, and infect a new tree. Here is more:
