Q.hydrangea “labelle” not blooming

I planted a very small “labelle” hydrangea with only one bloom 4 years ago. It has never bloomed again.
The leaves are lush and the plant grows larger every year. I don’t know if it is supposed to bloom on old or on new wood. I have given it fertilizer but I don’t know if I ever gave it phosphorous as you mentioned.
I never noticed if the plant dies down to the ground in winter.
Can you suggest some remediation?
