hydrangea care

We have a delicate hydrangea that was planted in full bloom early September by a landscape company. We were not given any instructions for care. I am now wondering what kind of fall and winter care it should have. The leaves have turned yellow and there are still blooms that look like typical dried hydrangeas on the plant. It actually looks like it could by dying. What kind of care should be given at this point? Any advice is welcome. It was a lovely plant when it was first planted and would like it to look that way again.
