How Do I Get Rid of Mushrooms in a House Plant?

I have recently moved this plant back home after an extended stay at another house. Long story short, I have had this plant for about 8 years, and have almost killed it numerous times. To the point there was one poor leaf remaining on it. I decided to take it to my boyfriend’s house; he has a patio door that gets sunlight most of the afternoon, and the plant was doing amazing. However, like I said, I brought “her” home 3 weeks ago and she doesn’t seem to be liking life here. Taking her back to his place is unfortunately not an option. She is now growing mushrooms, some of the leaves are turning yellow and falling off, and some of the leaves I am seeing little black specs on too. She was consistantly getting new growth, which she seems to still be getting, but on the one new sprout the tips of the leaves are blackish. I wish I could tell you the type of plant she is but I don’t know. She is kinda like a “tree”. There are 3 main stems and off of them branch the new branches/leaves. Some of the branches have 5 leaves and some have 6. In the beginning of bringing her back to life, the leaves were quite small, like 4 inches in length. As time progressed, the leaves have gotten a lot bigger, some up to 12 inches long. Please help me. This plant means a LOT to me and I seem to be killing it yet again.
