It was one of my favorite plants. Can you please tell me what I’m doing wrong and if it’s savable??

I am an optimist and think you can save your plant. Cut back the woodier stems to the lowest leaves. Fertilize if you haven’t done so in the past month. Especially in late fall/winter, getting good light and moisture conditions right is tricky. Always test the soil with your finger down an inch. Only water if the soil is bone dry at that level. Extra light while it is struggling may help so give it a bright window location. Winter sun where I live is pathetic and we have lots of cloudy days. I often move plants that do well in a NE window during summer to a south window at this time of year. Go by the conditions where you live. I hope your pot has drainage holes; without them, the risk of root rot is much higher. Here is a helpful article on growing polka dot plant.
