Growing carrots

Question

I’ve tried for several years now to grow carrots but have had no luck. I’ve tried sowing directly and also starting them before transplant. Every time, they germinate wonderfully and get to about 2 inches before shriveling up and dying. What am I doing/not doing that makes this keep happening?
