Grape Vines

I need to know how to move a 50 year old grape vine without killing it! It's been suffocated by a tree we had removed 3 years ago and is coming back slowly, but we need to move it to our backyard with out killing it! Please help! My name is Darbi and I really don't want to see my grapes get killed!
