Every year I get something attacking my apples, leaving red splotches on the apples, and I have leaves dying out at the end of branches. I do spray with an insecticide after blooms have fallen and again in midsummer. Please help with controlling these pests and can you tell me what I’m dealing with, please?

Without an image I cannot say for sure what issue you are dealing with.
My best guess from your description could be Cedar Apple Rust or Bitter Rot.
Inspect the leaves for any other identifying markings.
