I am trying to grow butterhead lettuce and iceberg. The butterhead has gotten very tall and "stalky," and no heads have formed. The iceberg has heads sort of, but very loose. Not nice compact tight heads. What am I doing wrong?
If it's getting tall and stalky, it may be bolting, and is usually brought on by high temps. Additionally, head lettuce requires a relatively low average temperature, particularly during the latter part of the growing cycle to form solid heads. My lettuce (Romaine and Butter) have begun to bloom. Is it still good? Should I pull the whole plant out by the roots?
This is called bolting. While the plants are still edible, their flavor will now be more bitter and won't taste as good. Most people choose to remove the lettuce plants once they start bolting. Here is more information on bolting:

I planted my 4x4 raised garden in mid-April (in middle TN). I used regular and organic soil. I have no vegetables at all. I get plenty of flowers, but no harvest. Not even my lettuce is growing! I have a tomato plant, green beans, cucumbers, squash and zucchini. I did have peas which were getting a couple of pods but those eventually died. I pulled those in mid-June and planted new pea seeds. Any help would be appreciated!

Some lettuce is naturally bitter, but heat or a lack of water can also make lettuce bitter. Bolting can also lead to biter tasting lettuce.
How many days should a lettuce and pak choi seedling be replanted? I have planted pak choi and lettuce seeds in a seed bed 2 weeks ago. I water them almost everyday to maintain a moisture soil.
