Would like to save this plant.
If the plant sits in soggy soil it can develop root rot and a bacterial leaf spot.
There is little to do if this is the case.
Check watering. If the the soil is dry this can also be the issue. To little water and the leaves will turn from yellow to brown.

My geraniums in a hanging basket have flowers but are losing all their leaves. What do I do?
They are likely underwatered. I would recommend increasing water. This article will help with how often to water containers:

How do I recognize when geranium goes into seed production? When do I collect them? Thanks, Debbie
If the geranium flower began to form pods these can be collected.
Gather the pods before they burst open.
Dry the pods completely and store in a sealed dark container in a cool area.
Sow the seeds in February for a the same years as they are slow growing. 13 to 15 weeks to flower after sprouting.
Remember if the plant is a hybrid the seeds may be sterile and would most likely not be true to the parent plant.
Cuttings are the recommended method of propagation for Geraniums.

I live in San Diego area. Some main stems are close to 1 inch in diameter. Can we save this plant and give it a more pleasing appearance? If so, how do we do it?
Sounds like an amazing specimen.
Here are a couple of links that will help you get your plant pruned.

Why do my geranium flowers turn dark and petals fall off (and when I pinch them off, no new flowers grow)? I used potting soil w/fertilizer and fertilize occasionally as well. Can it be too much water or not enough? Thank you.
sounds like your spoiling your geraniums too much water, I never feed mine and they are very happy
My cranesbill geraniums have grown really well with lots of foliage but they haven't had a single flower on them. It is the first season.
Did you fertilize? Oftentimes, this is a result of too much nitrogen in the soil and not enough phosphorus. Try adding some phosphorus rich fertilizer (Bloom booster) or bone meal to the soil around your plants.
Does one take the entire flower stem off when the blossums are faded, or just the faded blossums, leaving the rest of the flower stem to shrivel up?
Yes, you just pinch the spent flowers off. And, by pinching them off, you encourage the plant to bloom more. This article will explain about it: https://www.gardeningknowhow.com/ornamental/flowers/fgen/deadheading-flowers.htm
